Blepharoplasty (Eyelid Surgery)

What is this treatment?

During blepharoplasty, the surgeon removes excess skin and may reduce or reposition fat. An upper eyelid incision usually follows the natural crease. Lower eyelid access may be below the lashes or through the inside of the lid.

However, surgery does not correct every dark circle. Pigmentation and thin skin, for example, need separate assessment. Ptosis means a low eyelid margin and requires assessment of the lifting muscle. The NHS eyelid surgery information explains general options.

Who may be suitable?

• Adults whose upper eyelid skin affects vision or daily comfort.
• People with lower eyelid bags or increasing skin laxity.
• People exploring surgery for inherited eyelid features.

However, dry eyes, glaucoma, thyroid eye disease and uncontrolled diabetes need further assessment. Also disclose bleeding problems and medicines. Do not stop blood thinners on your own.

How is it performed?

1. Examination: The surgeon checks extra skin, eyelid movement and the eye surface. Visual field testing may be necessary.
2. Planning: First discuss which eyelid needs treatment. Then consider anaesthesia and incision placement.
3. Surgery: The surgeon adjusts the planned tissues and closes the incision.
4. Follow-up: Going home depends on your condition and anaesthesia. The team explains wound care and follow-up.

Recovery

Swelling, bruising and a gritty sensation may occur initially. Use recommended cold compresses and prescribed drops as instructed. You may also receive advice to rest with your head raised.

Many people plan about two weeks away from work. However, bruising and scars may take longer to settle. Ask about stitches, makeup and contact lenses at follow-up. Follow individual advice about exercise and driving.

Risks and side effects

Possible complications include bleeding, infection, noticeable scars and asymmetry. Dry eyes, incomplete eyelid closure or an outward-turning lid can also develop. Some problems need further treatment; surgery cannot guarantee a particular result.

Seek emergency care for sudden reduced vision, severe eye pain or rapidly increasing swelling on one side. Do not wait for a routine appointment. Rare bleeding behind the eye can threaten sight. Also promptly report increasing redness or discharge to the treating team.
