Blepharoplasty addresses excess skin and, when needed, fat; ptosis surgery addresses the mechanism that lifts the eyelid margin. Brow descent can look similar, so the approach is chosen after examination. Sudden one-sided drooping with double vision, unequal pupils or weakness needs urgent assessment.

In a transconjunctival approach, the surgeon reaches the fat through the inner surface of the lower eyelid, without an external skin incision. It does not remove excess skin. A different or combined surgical plan may be needed when excess skin or eyelid support is also a concern.

No. Fat grafting surgically harvests your own fat from a donor area and transfers it to the face; the amount that remains can vary. Hyaluronic acid filler is an injectable, temporary gel, with different options, recovery and risks.
