Botulinum toxin: expression lines and lines visible at rest
Botulinum toxin temporarily reduces activity in selected muscles; it does not fill every crease.

What should the assessment clarify?
Assessment considers the face at rest and during expression, earlier procedures and eyelid or brow position. A line is not a prescription: ask whether muscle activity is relevant and what treatment is unlikely to change. T1T2
| Concern | What is assessed | Possible relevance | Important limit | Question to ask |
|---|---|---|---|---|
| Expression line | Appearance during movement | Temporary muscle relaxation may be relevant | Response and duration vary | What movement are we discussing? T2 |
| Line at rest | Appearance without expression | Assessment of whether the proposed indication fits | Not every resting crease will disappear | What change is realistic? T1 |
| Volume or contour concern | Whether a filler question is involved | A different material and risk discussion | Filler is not botulinum toxin | Is this a different concern? F1 |
This comparison helps frame consultation, not select an injection site or dose. The US product label concerns particular indications and products; it does not establish suitability for every facial line or make different preparations equivalent. T1T2
What are the options and limitations?
Options include no treatment or an individually assessed procedure. Ask why the product matches your concern, what may remain unchanged and what follow-up involves. Take time to decide. T1C1
Effects are temporary and individual response varies. Reducing muscle activity does not guarantee a particular expression, symmetry or disappearance of all lines. Published product information should accompany discussion of expected benefit and relevant risks. T1T2
How should I prepare for the consultation?
Bring your medication list, previous toxin product and treatment dates if known, allergies and medical history. Mention swallowing or breathing problems, neuromuscular conditions, pregnancy or breastfeeding. Do not stop prescribed medicines yourself; discuss interactions and whether treatment should be deferred. T3T1
Which risks need discussion?
Local pain, swelling, headache and eyelid or brow drooping can occur; dry-eye problems are also relevant. The label warns that toxin effects may spread beyond the injection area. Infection at the proposed site and relevant hypersensitivity require specific assessment before treatment. T1
How should follow-up be organised?
Follow-up should assess symptoms and the agreed goal before any further treatment is considered. Report unexpected eyelid changes or eye discomfort. A temporary effect does not justify an automatic repeat schedule, product change or extra injections without reassessment. T1T2
What else should I ask?
Does botulinum toxin add volume?
No. Its cosmetic mechanism temporarily relaxes selected muscles; it does not add facial volume. This distinction matters when a concern is described simply as a wrinkle. T2
Are different products’ units interchangeable?
No. The BOTOX Cosmetic label states that its potency units are not interchangeable with those of other botulinum toxin preparations. Do not compare offers by treating units as equivalent. T1
Does approval cover every facial area?
No. Authorizations are product, indication and jurisdiction specific. A US label is not confirmation of Turkish authorization, local availability or suitability for another site or product. T1
Can a natural expression be guaranteed?
No. Explain which movements you want to preserve and ask about limitations and drooping risk. Neither a photograph nor the word natural guarantees a result. T1C1
What information should I bring?
Bring a complete medication and procedure history, including other toxin treatments and muscle relaxants. Ask which conditions or symptoms could change the decision; leave medication changes to the prescribing clinician. T1T3
