Ultrasound-guided filler dissolution is the process of evaluating previously applied hyaluronic acid–based fillers under ultrasonographic imaging and dissolving them with the enzyme hyaluronidase. Hyaluronidase breaks down the natural structure of hyaluronic acid and acts only on this type of filler. It is preferred in situations such as an unwanted overfilled appearance, asymmetry, displaced filler, or suspected vascular compromise, with the aim of gradually reducing or fully dissolving the filler.

Ultrasound plays an important role in this process. High-resolution imaging maps the location, depth, and proximity to vascular structures of the filler beneath the skin. Particularly in anatomically sensitive, vessel-rich areas such as the periorbital region, ultrasound supports safety by allowing both the true position of the filler to be seen and the enzyme to be applied in a targeted, controlled manner. In this way the procedure is planned with imaging information rather than blindly.

Nasıl yapılır?

1. Ultrasonographic evaluation: The periorbital area and treatment site are examined with high-resolution ultrasound; the location, depth, and proximity to vessels of the filler are mapped.
2. Planning: Based on the imaging findings, the physician determines the area to be dissolved, the amount of enzyme, and the depth of application.
3. Preparation: The area is cleansed; when needed, a topical anesthetic cream is used for comfort support.
4. Hyaluronidase application: The enzyme is delivered into the targeted filler area with a fine needle in a controlled manner, guided by ultrasound when required.
5. Review: The area is assessed after the procedure, and a follow-up session is planned where necessary.

• First hours: Mild redness, swelling, or tenderness may occur at the site; it usually subsides during the day.
• First day: Most people can return to daily life the same day; avoiding massage and pressure to the area is advised.
• First few days: Temporary swelling and, rarely, mild bruising recede and resolve.
• 1-2 weeks: The effect of dissolving the filler becomes clearer; a follow-up examination is planned if the physician considers it necessary.
• Later period: If a new application is being considered, an appropriate interval is observed as advised by the physician.

Ultrasound-guided filler dissolution is generally a well-tolerated procedure; nevertheless, as with any medical application, some temporary effects and rare risks may occur. The area and the application are evaluated and planned by the physician.

• Temporary redness, swelling, or tenderness at the site
• Rarely, mild bruising
• A rare possibility of an allergic reaction to hyaluronidase
• The dissolving effect may vary from person to person and by filler type
• In some cases the procedure may need to be repeated
